Título:Single lesion subpolar lepromatous leprosy and its possible mode of origin.

Resumo:Three case reports of patients with a single, nodular, subpolar lepromatous skin lesion, one on the left elbow, another on the posterior aspect of the left leg, and the third on the extensor ulnar aspect of the right forearm, are presented. The lesions, clinically and histopathologically, resemble lepromas which develop at the site of experimental inoculation of Mycobacterium leprae in armadillos. These are sites on the body which are likely to be traumatized. With the distinct possibility of the presence of viable M. leprae in the soil of Louisiana and Texas from wild armadillos with the natural disease, it is suggested that these three patients acquired the infection from the environment and had inoculation lepromas^ien.

Resumo:In this presentation an attempt has been made to describe the nine-banded armadillo as an animal model, probably the only one in which lepromatous leprosy similar to that found in humans can be experimentally produced. Some unique features of the physiology of the animal are mentioned. The pathology and the microbiology of leprosy in the armadillo are described in detail. The discovery of lepromatous leprosy in the wild armadillos in the southern parts of United States, the transmission of disease among them through trauma and thorn pricks and the pathogenesis of the disease are presented. The impact of leprosy in the wild animals may have on human leprosy is discussed. (AU).

Resumo:1- A xanthoma tuberosum lesion which appeared in a lepromatous patients has been studied, using electron microscopic and histopathologic procedures. 2- The same cells in this lesion exhibit two structures characteristic of lepra cells and xanthoma cells. The coalescent pinocytotic droplets, opaque droplets, foamy structures and leprosy bacilli belong to those of lepra cells, whille onion-like structures and transparent vacuoles are characteristic of xanthoma cells. 3- The interpretation of the electron micrographs indicates that the cells in this lesion deviate from the normal lipid metabolism, especially that of cholesterol. 40 Comparative observations between the transparent vacuole and the foamy structure indicate that the latter does not contain sterol derivatives. Resumo:La existencia de un defecto específico en la respuesta inmune mediada por células ha sido señalada en la lepra lepromatosa. Se sugiere la posibilidad de uq el interferon (IFN) gamma influya em el dominio del espectro inmunológico lepromatoso. Se hace un estudio para demostrar si el IFN gamma como coadyuvante de la terapéutica específica es capaz de transformar o modificar el efecto inmunológico en la lepra lepromatosa. Se seleccionarán 20 pacientes vírgenes de tratamiento o con menos de dos años de terapéutica específicas distribuidos de acuerdo a tabla aleatoria en dos grupos que reciben IFN gamma o placebo además de la quimioterapia especifica. Antes del tratamiento y al final del mismo se realizarán los siguientes estudios: histopatológico, inmunológico y bacteriológico. Se presenta comunicación preliminar de acuerdo a corte realizado en los dos pacientes que culminaron su estudio: uno tratado con IFN gamma y otro que recibió palcebo, siendo la respuesta notablemente superior en el primero desde los puntos de vista clínico, bacteriológico e inmunológico, llegando a negativizar la detección de bacilos. Resumo:Interleukin-12 receptor beta 1 ( IL12RB1), interleukin-12 receptor beta 2 ( IL12RB2), and interferon gamma receptor 1 ( IFNGR1) perform important roles in the host defense against intracellular pathogens such as Mycobacteria. Several mutations within their genes have been confirmed as associated with increased susceptibility to mycobacterial infection. However, the association between mutations of the IL12RB1, IL12RB2, and IFNGR1 encoding genes and lepromatous leprosy has not been studied. This study screened for polymorphisms within IL12RB1, IL12RB2, and IFNGR1 encoding genes in the Korean populations using polymerase chain reaction (PCR)/single-strand conformation polymorphism (SSCP) DNA sequencing assay, and an association study was performed using the missense mutations of 705 A/G (Q214R), 1196 G/C (G378R), 1637 G/A (A525T), and 1664 C/T (P534S) of the IL12RB1, 83 G/A (V14M), and 1443 T/C (L467P) for the IFNGR1 encoding genes. There were no differences in the genotype and allele frequencies of IL12RB1 and IFNGR1 genes between 93 lepromatous leprosy patients and 94 control subjects. In conclusion, missense mutations of 705 A/G (Q214R), 1196 G/C (G378R), 1637 G/A (A525T), 1664 C/T (P534S) of the IL12RB1, 83 G/A (V14 M), and 1443 T/C (L467P) of the IFNGR1 encoding genes have no association with the susceptibility to lepromatous leprosy in the Korean population.
